Catastrophic Leave Donations Sought

Please help Jan Hillman, Associate Health Physicist, California Department of Public Health (CDPH).  One year ago, Jan was taken to the hospital with a life-threatening medical emergency.  Ms. Hillman survived the ordeal, and bravely endures a slow recovery.  Her medical condition has left her very short of leave time.

Ms. Hillman’s leave has been approved through February 2012.  As of November 1, 2011, she only has 16 hours on the books. Any donations would be appreciated.  Attached is the CDPH Catastrophic Leave donation form.  Once complete please submit to Yvonne Ronan, Radiologic Health Branch 916-440-7949, fax 916-341-7198, or Yvonne.ronan@cdph.ca.gov.

If a non-CDPH member is able to make a donation, please obtain a copy of your department’s leave donation form.  Submit the form to your department’s personnel office and direct the department to provide the authorization to Yvonne Ronan as described above.  Please inform Ronan of your donation when the form is submitted.
