If you need to send CAPS a
new address, e-mail address, phone number, or other change, please
submit your updated information
here, using
our secure server.
You may also contact the CAPS office nearest you by phone or email. Offices are listed below. If you would like to change change the beneficiary on your Group Insurance coverage, please print and fill out a Beneficiary Designation Form here, and either mail it or fax it to the CAPS Insurance office listed below. (Unfortunately, we cannot accept this form electronically because it requires a signature.)
